Question 1

Peer review assessment

In the given elog there is a long case with 2 short cases.

  • All the 3 cases are presented well with clear theory explanations .History taking was excellent in the long case. Diagnosis of the long case 
  • Case of rheumatoid arthritis was presented with all the reasons for the treatment given
  • The captured data is complete,correct and useful to analyze the diagnosis and treatment given.
All three cases are presented well with photographs which are helpful in easy understanding.

Problem list

-Painful joints associated with localized edema and limited movements 
-Burning sensation of eyes
-Early morning stiffness of joints lasting for approximately 1 hour
-Frothy urine with decreased urine output
 -Glomerulonephritis

Treatment 

Free water restriction for hyponatremia
Tab prednisolone p/o 20mg OD
Tab febuxostat p/o 80mg OD

Short case

Problem list 

2 month history of progressive asymmetric involuntary movements of his right index and middle fingers.

 stiffness in his wrists

he also says that his walking has become difficult with small, short steps and a forward stoop, and he feels that although he weighs 60 kgs. he feels like it weighs 100 kgs. 

morning erections since 2 months and also reports a loss of sexual desire.

has an immediate urge to defecate when he has tea and sometimes goes 2 to 3 days with constipation
speaking in a monotonous drab since 2 months. 

Treatment
1. Tab. Syndopa Plus 125 mg QID
2. Tab. Syndopa 125 mg CR OD
3. Tab. Telma 40 mg OD

Short case 2

Problem list

Itchy Ring leisons over arms ,abdomen ,thigh and groin since 1 and half year .

-Purple stretch marks all over abdomen ,lower back ,upper limbs ,thighs since 1 year .

-Abdominal distension and facial puffiness since 6 months.

- Pedal edema since 3 months.

- Low back ache since 3 months .

- Feeling low , not feeling to talk to anyone.

- Weight gain and decreased libido since 3months.

- Loss of libido and erectile dysfunction since 2 months .


Treatment 

Ointment AMLORFINE 

FUSIDIC ACID CREAM.

SALINE COMPRESS OVER LEISONS


Question 3

Appropriate diagnosis was done according to the investigations done .
Reason for the provisional diagnosis was also explained to the par
Treatment given was also appropriate
But in the long case the diagnosis was somewhat 
difficult in distinguishing glomerulonephritis and rheumatoid arthritis
