 I have been given the following assignment to analyze , and review, in an attempt to understand the topic of 'Patient clinical  data analysis' to develop my competency in reading and to comprehending clinical data including history, clinical findings, investigations and diagnosis,

Case

I have given the case of a person with wernickskorsekoff syndrome who is chronic alcoholic.He had two episodes of seizures .After his last alcohol started talking and laughing to himself. unable to getup by himself from bed 
he is conscious  but not coherent.

KINDLING ALCOHOL WITHDRAWL

He also had renal failure ,due to generalized dehydration.He had a foot ulcer due to peripheral neuropathy along with diabetic neuropathy.In this case the explanation is very much simplified and for each sign there is a probable reason given.

Provisional diagnosis was correctly made. Discharge summary also includes future care and treatment plan. The MRI scan done to confirm cervical myelopathy rules out .The photographs are well seen and explained where we can appreciate the symptoms of the patient.It is useful in easy way of understanding the case.

A CASE OF A 45 YEAR OLD MALE WITH ACUTE KIDNEY INJURY ON CHRONIC KIDNEY DISEASE (HYPERTENSIVE NEPHROPATHY) WITH URAEMIC ENCEPHALOPATHY.

General and systemic examinations are well.done
But the treatment should have been eloberated for better understanding. 

Question 5

I felt that it s very much useful as I can learn many new things from this ,not only the related subject but also the presentation. But I thought I may require some more guidelines to do it perfectly.
